Home Care Packages

The Home Care Packages (HCP) are designed for those with more complex care needs that go beyond what the Commonwealth Home Support Programme can provide.

FCCVi HCP services may include, but are not limited to:

Nursing
Help with impairments or continence
Podiatry, physiotherapy, and other therapies
Aids to stay independent
Bath, hygiene and grooming
Meals and food preparation
Home or garden maintenance
Transport: community access
Cleaning, laundry, and other chores

How to know if you're eligible to apply for HCP support funding?

You are 65 years old and above
You notice a change of care needs
You require help to remain independent
You have been diagnosed with a medical condition or reduced mobility

Our Home Care Packages are government-subsidised programs that provide coordinated care and services to help individuals continue living independently as they age. There are four levels of Home Care Packages to help meet the different levels of ageing-related care needs. The assessment stage will determine what level you will be eligible for.
